2. Original and/or certified true photocopy of Medical Abstract signed by the attending physician with license number
3. Prescription or Request with printed name, license number and signature of the attending physician
4. Official Price Quotation (*If Implant - 3 Quotations from suppliers)
5. Endorsement Letter from the Hospital or Suppliers (Acceptance of Guarantee Letter)
6. X-ray or Radiology Result (for Implant)
7. Photocopy of ID card of patient and assisting direct relative
