2. Original and/or certified true photocopy of Medical Abstract signed by the attending physician with license number
3. Original itemized Statement of Accounts / Hospital Bill duly signed by the authorized person of the hospital (with all necessary deductions - PhilHealth, Senior Citizen discounts, etc.)
4. Promissory Note (if outpatient / discharged)
5. Endorsement letter from the hospital (acceptance of Guarantee Letter) - Credit and Collection Department
6. Photocopy of ID card of patient and assisting direct relative

***All documents must be filed within 30 days after discharge***

"Naku de Lima, huwag mong tinu-twist ang mga istorya ng mga ginawa noon ni Mayor kumpara sa ginagawa niya ngayon bilang Pangulo! Ang pagmamagandang-loob noon ni Mayor Duterte sa mga NPA ay hindi nangangahulugan na sinusuportahan niya ang mga ito na labanan ang gobyerno! Likas lang talaga kay Tatay Digong na maging mapag-kawanggawa anumang kulay mayroon ang isang indibidwal na Pilipino. May nabalitaan ba kayo dati na ina-ambush ng mga NPA ang mga kapulisan at kasundaluhan sa Davao region? May mga terorismo ba silang ginagawa noon sa balwarte ni Tatay Digong? Wala, hindi ba?! Naging payapa at maunlad ang Davao City dahil sa karisma, pagiging sinsero at diskarte ni Tatay Digong! Nitong bagong upo ang Pangulong Duterte, ilang beses bang inilahad ng Pangulo ang pakikipagkasundo sa mga NPA upang wakasan na ang ilang dekada ng kanilang rebelyon at upang magkaroon tayong lahat ng kapayapaan, kaunlaran at makabuluhang pagbabago sa ating lipunan? Maraming beses, de Lima!  Ngunit sa kasamaang-palad ay hindi ito nasuklian ng tama ng mga Komunista! Sino ba naman ang hindi magsasawa sa ganiyang sistema ng pakikipagkasundo sa isa't-isa? Hindi sinsero ang isang panig!

News of the Day: "Sandra Cam vows to clean up PCSO"

No sooner had former “jueteng” whistle-blower Sandra Cam set foot in the Philippine Charity Sweepstakes Office (PCSO) than she found a case to dig into.

Cam, newly appointed PCSO board member, said at least 70 of the 84 authorized agent corporations were using Small Town Lottery (STL) as a front to the illegal number games jueteng.

“Out of the 84 STL outlets in the country, around 70 are being controlled by gambling lords. They are using STL for their bookies. Only 14 are operating legally,” she said in a press conference during her second day in office.

Cam figured in an outburst with airport workers earlier this year when she was not accorded VIP treatment after her failure to present a government ID. “I don’t have a government ID but in three months, I will be a member of the Cabinet,” she had said.

Cam and lawyer Bong Suntay, former Quezon City councilor, were both sworn in on Dec. 6 as the newest directors of the PCSO board.

“The President wants to give the full assistance to the masses, but we cannot give all; we cannot give the best this government wants because we are shortchanged. There is only a (percentage) that our government gets. I am not against anything, I can be one of them but only in the right direction,” Cam said.

“Of all the board members here, I am the only one capable to expose illegal STL operations. This is my forte … This is my first time to serve the government. (My goal) is to eradicate corruption and this agency has a lot of those … I am thankful for the trust and confidence President Duterte gave me … I am very particular on the games,” she said.

According to Cam, the top provinces where jueteng is rampant are Bicol, Bulacan, Pampanga, Nueva Ecija and Pangasinan.

She said 10 whistle-blowers who were professionals would serve as her confidential agents who would expose irregularities in STL. Each agent would be paid P18,000 monthly to check the agency’s operations and illegal activities nationwide.

Sandra Cam appointed to PCSO; Abella swears in as DFA usec

President Rodrigo Duterte has appointed whistleblower and staunch supporter Sandra Cam to the Philippine Charity Sweepstakes Office (PCSO).

She told reporters in Malacañang that she had been appointed as a member of the PCSO board of directors.

She is scheduled to take her oath Wednesday along with two other PCSO board appointees, other government appointees, and officers of private organizations before Duterte.

Former presidential spokesperson Ernesto Abella will also take his oath today as an Undersecretary of the Department of Foreign Affairs. — MDM, GMA News
