Requirements for Hospitalization
(All documents must be original & updated)

1. Personal Letter Request addressed to:

SANDRA M. CAM

Note: please indicate your contact number

2. Original and/or certified true photocopy of Medical Abstract signed by the attending physician with license number
3. Original itemized Statement of Accounts / Hospital Bill duly signed by the authorized person of the hospital (with all necessary deductions - PhilHealth, Senior Citizen discounts, etc.)
4. Promissory Note (if outpatient / discharged)
5. Endorsement letter from the hospital (acceptance of Guarantee Letter) - Credit and Collection Department
6. Photocopy of ID card of patient and assisting direct relative

***All documents must be filed within 30 days after discharge***
